The German side had agreed a fee of around &pound21m for the player's sale earlier this week.

According to German sports daily Bild, the Ghanaian international has undergone a series of medical tests at Chelsea, and the transfer could be announced in the next 24 hours.

Rahman is likely to slot in as back-up to Cesar Azpilicueta at left-back for the ongoing league campaign.
