Manchester City attacker Raheem Sterling is set to undergo his medical with Chelsea following a transfer agreement.
The England international bagged 17 goals and nine assists for City last term, but was largely in-and-out of the line-up.
With a year left on his contract, he has decided to pursue a new challenge with Chelsea, who will pay £45m up front.
The 27-year-old has recently been permitted to travel to the English capital, and his medical is scheduled to happen soon.
Sterling is set to become Chelsea's first signing of the transfer window, and there could be several more to come this summer.
The Blues are also interested in re-signing Nathan Ake from City, but they are yet to negotiate a suitable transfer package.
The Premier League champions are willing to part ways with Ake, provided they receive a fee of around £41m from Chelsea.
The west London giants have already agreed personal terms with the Dutchman, who started his senior career with them.
