Ziyech has experienced a tough season with the Blues and he is currently on the fringes of the first-team squad.
It is likely that he could leave in the New Year and the forward has a particular preference to join the Rossoneri.
According to Calciomercato, Ziyech is determined to join the Serie A champions after missing out last summer.
He is hoping to impress them with his performances at the World Cup such that they can buy him outright in January.
The Moroccan has struggled for game time at Chelsea despite the change of manager. He is said to be frustrated on his role.
The Blues did not receive any permanent proposals for him last summer and it remains to be seen whether that changes.
Aside from Ziyech, there are suggestions that Christian Pulisic may also attempt to leave the club at the turn of the year.
